Use when an account, inquiry, or collection resulted from identity theft and you have an FTC Identity Theft Report or police report.
How to use it
- Create an IdentityTheft.gov report.
- Attach proof of identity and the identity theft report.
- List each fraudulent item clearly.
- Place a fraud alert or credit freeze if appropriate.
